چکیده

A compromised skin barrier plays a major role in many dermatoses including irritant and allergic contact dermatitis, atopic dermatitis, dry skin, aged skin, xerosis, rosacea, and acne [1–5]. Many of these conditions share common defects in the skin barrier and an association with inflammation [4,6]. The knowledge we have regarding specific lipid deficiencies, pH aberration, inflammation, irregular calcium gradients, and susceptibility to contact sensitization can be leveraged to address many aspects of the disrupted skin barrier. By addressing these major points of vulnerability the skin’s inherent ability to heal itself can be optimized for skin barrier repair.

Pathogenesis of Atopic Dermatitis: Current Paradigm

Atopic dermatitis (AD) is characterized by skin inflammation, barrier dysfunction and chronic pruritus. In this review, recent advances in the pathogenesis of AD are summarized. Clinical efficacy of the anti-IL-4 receptor antibody dupilumab implies that type 2 cytokines IL-4 and IL-13 have pivotal roles in atopic inflammation. The expression of IL-4 and IL-13 as well as type 2 chemokines such a...

Incontinence-associated Dermatitis: Adverse Event Sequelae, Cost and Prevention

Incontinence-associated dermatitis (IAD) is a common problem in patients with faecal and/or urinary incontinence. Urine alters the normal skin flora and increases permeability of the stratum corneum and faecal enzymes on the skin contribute to skin damage. Faecal bacteria can then penetrate the skin, increasing the risk of secondary infection. However, IAD can be prevented and healed with timel...

Skin Barrier Disruption - A Requirement for Allergen Sensitization?

For at least half a century, noninvasive techniques have been available to quantify skin barrier function, and these have shown that a number of human skin conditions and disorders are associated with defects in skin permeability. In the past decade, several genes responsible for skin barrier defects observed in both monogenetic and complex polygenic disorders have been elucidated and functiona...

Management of Patients with Atopic Dermatitis: The Role of Emollient Therapy

Atopic dermatitis is a common inflammatory skin disorder that afflicts a growing number of young children. Genetic, immune, and environmental factors interact in a complex fashion to contribute to disease expression. The compromised stratum corneum found in atopic dermatitis leads to skin barrier dysfunction, which results in aggravation of symptoms by aeroallergens, microbes, and other insults...
